To book any PMU/Permanent Make-up appointment, you must answer these prequalifying questions truthfully.

All answers should be a "NO" to be pre-qualified. If any of these answers are "YES",

please do not book online and call NadyB Ink Bar. You must be a qualifying candidate

for any of these services and they must be safe to be performed on you.

Deposits are non-refundable and non-transferable.
